General Requirements
- Pass vision test
- Pass written knowledge test
- Pass road skills test
- Residency: Must be a Wisconsin resident with proof of residency such as utility bills, bank statements, or rental agreements
- Parental consent: Required for applicants under 18 years old - parent, guardian, or adult sponsor must sign the application
- Other: For learner permit: must be enrolled in driver education (under 18). For provisional license: must hold permit for 6 months, complete 50 hours of supervised driving (10 at night), be violation-free for 6 months, and pass road test. Provisional license has restrictions for first 9 months including passenger limits and night driving restrictions (midnight to 5am). Full license at age 19 if maintaining clean driving record.
